Community Referrer, Going Local (funded initially for 1 year)

Two roles - one based in Worthing (37 hours per week) and one based in Adur 4 days per week (29.6 hours per week) Please stipulate your preferred location

Place of work: Based in GP surgeries in Adur and Worthing, remote worker.

Then a Community Referrer role within our social prescribing project, Going Local, maybe the perfect job for you!

Going Local has been operating in Adur and Worthing since 2016. Over 2,000 people have been referred to find out ‘what matters to them’, as opposed to ‘what is the matter with them’, enabling them to deal with issues that often include housing, mental health, money advice, lifestyle issues, loneliness, volunteering and addiction support, amongst many others.

We’re looking for new Community Referrers to join the team.  These two new roles will be managed by Adur and Worthing Councils.  Being an integrated part of the GP practices and the wider Primary Care Network family, the post-holder will be expected to work at the surgeries, be comfortable in being shared across a number of sites and confident in building rapport with practice staff at all levels.

If you’re passionate about people and have excellent core communication skills for working with our service-users and our many partners, this could be the job for you! 

You’ll need to be good at prioritising, efficient with your time and a good navigator - of both people and agencies.  And tenacious - we need clear case-notes, good diary management and attention to detail.

You must have client-facing experience as well as sound motivational interviewing skills and be a great ‘active’ listener.  We’ll help you develop these and other skills, making sure we’re the best for the people we’re supporting in our local communities and that your personal development continues to flourish.
